I am aware of the policies regarding borrowing electronic devices from the library. I will take care of this device by doing the following:
Store it in a secure location so that it will not be lost or stolen
Store and use it carefully so that it will not be damaged through action OR inaction
Ask questions of the Library Team or the Help Desk if I need any assistance
Return it promptly to a Library Team member so that others may also use it (do not leave it on the circulation desk)
I acknowledge that I am taking personal and financial responsibility for this device while it is checked out to me. DEVICES CAN BE CHECKED OUT FOR UP TO TWO WEEKS. IF NOT RETURNED WITHIN THAT TIME, YOU WILL BE CHARGED FOR ITS REPLACEMENT COST.
